If you’ve been off sick for more than 7 days

If you are unwell and cannot work for 7 days or more (including bank holidays and weekends) then your employer may require a Fit Note (sometimes called a sick note) from the GP.

You can request a Fit Note from the surgery by using the link below and clicking on Admin Request. 

Your Fit Note covers all days stated on it and if you require more time off from work, you will need to request another Fit Note once the current one has ended.

You do not need to see a Health Care Professional before returning to work when your Fit Note has ended

If you’ve been off sick for 7 days or less

If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. We call this self-certification.
